The Government of Moldova instituted several policy and legal initiatives to expand access to adolescent sexual and reproductive health and rights (ASRHR) services from the early 2000s onwards. The strong legislative and policy environment, coupled with strengthening data and interventions aimed at increasing service delivery, have led to improved ASRHR outcomes in the last decade, including increasing use of contraceptives and decreasing teens pregnancy rate. Adolescents played a crucial role in designing and delivering the interventions.\r\n\r\n<strong>Sexual and Reproductive Health &amp; Rights Challenge<\/strong>\r\n\r\nMoldova has a universal health care system that aims to provide access to basic health services for all its citizens. The period following the dissolution of the Soviet Union in the 1990s led to significant social, economic, and health system disruptions in Moldova. Adolescents and youth were particularly affected with increases in sexually transmitted infections and early and unintended pregnancy. Between 1994 and 1996, one in five births were to mothers under the age of 20 years. The incidence of HIV among adolescents in Moldova has been rising and adolescents aged 15\u201319 years accounted for 14.4% of HIV-infected people in 2003. Since the early 2000s, the Ministry of Health of Moldova prioritized re-orienting the public health system, including prioritizing meeting ASRHR needs. The country introduced a mandatory health insurance (MHI) system in 2004 to improve the efficiency and transparency of health financing, prioritizing primary care. Approximately 88% of the population was covered by mandatory health insurance in 2021.\r\n\r\n<strong>Intervention<\/strong>\r\n\r\nTo address ASRHR issues, the Republic of Moldova developed and implemented youth friendly health services (YFHS). This involved a multi-disciplinary approach with active involvement of youth. YFHS were implemented with specific actions in the health system building blocks:\r\n<ul>\r\n \t<li><strong>Leadership and governance:<\/strong>\u00a0Adolescent health and access to YFHS was a priority in the National Strategy for Reproductive Health 2005-2015, National Health Policy 2007\u20132021, and continues to be a priority for the National Health Strategy 2030. According to health law and reproductive health law, adolescents can access health services, including SRH services, without parental consent from age 16 years, or younger in cases where parents cannot be reached, and life-saving services are needed.<\/li>\r\n \t<li><strong>Health system financing:<\/strong>\u00a0Health insurance for children, students, and pregnant women is paid for by the state. A basic package of services is available free for all. The national health insurance company has been funding Youth Friendly Health Centres (YFHC) since 2008 ensuring free access for all young people aged 10\u201324 years and for vulnerable youth up to 35 years of age.<\/li>\r\n \t<li><strong>Service delivery:<\/strong>\u00a0YFHS are provided by a network of 41 YFHC \u2013 Youth Klinics (YK) in public primary health care institutions in all municipalities and districts of Moldova, based on Youth Friendly Health Services Quality Standards with a special focus on rural youth, vulnerable and at-risk adolescents. Services include outreach (YK Mobile) and online counselling (YK Support line).<\/li>\r\n \t<li><strong>Health workforce:<\/strong>\u00a0Adolescent healthcare training courses\/modules were incorporated into preservice curricula at the medical university and nursing and midwifery schools. Collaborative learning sessions are organized regularly by all YFHC\/YKs.<\/li>\r\n \t<li><strong>Health information systems:<\/strong>\u00a0In addition to the national health management information system (HMIS), a separate confidential component \u2013 Youth Klinic-Informational System \u2013 was developed and implemented for YFHCs\/YK to monitor activities of YFHS.<\/li>\r\n \t<li><strong>Medical products:<\/strong>\u00a0Since 2017, contraceptive products and rapid tests for HIV, syphilis, hepatitis are procured by the State and distributed by family doctors, reproductive health offices, YFHCs. It has been possible to procure medication for medical abortion for adolescents in one YFHC since 2012 and in all YFHCs since 2022\u20132023.<\/li>\r\n<\/ul>\r\n<strong>Outcomes<\/strong>\r\n\r\nThe strong legislative and policy environment, coupled with strengthening data and interventions aimed at increasing service delivery have led to improved ASRHR outcomes in the last decade. Access to YFHS for 10\u201324 year olds increased from 5% in 2011 to 38% in 2023. Outreach efforts resulted in a higher number of vulnerable service users and male beneficiaries accessing the services. Contraceptive pill use among sexually active adolescents increased from 6% in 2014 to 13% in 2022, and the majority of sexually active adolescents (74%) continue to use condoms. The adolescent fertility rate declined from 34.2 per 1000 women aged 15\u201319 years in 2015 to 23 per 1000 women in 2022. At the same time points, abortion rates declined from 11.7 per 1000 women aged 15\u201319 years to 5.9\/1000. HIV incidence among 15\u201324 years olds remained unchanged during the last decade and is more than two times lower than HIV incidence in the general population.\r\n\r\n<strong>Challenges<\/strong>\r\n\r\nChallenges for YFHS include political, social and humanitarian instability, underfunding, limited access for young people from rural areas, and insufficient mechanisms to motivate medical staff in hospitals and family doctors to apply the friendly approach to adolescents seeking medical care. Furthermore, the underdevelopment of school health services and lack of implementation of a complex health education program present obstacles to ensuring universal access to friendly health services for adolescents in the Republic of Moldova.\r\n\r\n<strong>Lessons<\/strong>\r\n<ul>\r\n \t<li>Legislative and policy reforms expanded the range of services that adolescents and young people could access, removing critical structural barriers and expanding geographical coverage.<\/li>\r\n \t<li>Reduction of service charges for adolescents, specifically within financing and benefits packages, and accessing services without parental consent, help remove barriers to ASRHR.<\/li>\r\n \t<li>Integrating ASRHR into existing pre-service health training institutions helps build a large cadre of providers who could offer adolescents and young people seamless services without the need for specialized training.<\/li>\r\n \t<li>Integrating ASRHR into essential packages of YFHS created a \u2018one-stop shop\u2019 for key SRHR and mental health services for adolescents. Conform legii privind s\u0103n\u0103tatea \u0219i legea cu privire la s\u0103n\u0103tatea reproductiv\u0103, adolescen\u021bii pot accesa servicii de s\u0103n\u0103tate, inclusiv servicii de s\u0103n\u0103tate sexual\u0103, f\u0103r\u0103 consim\u021b\u0103m\u00e2ntul p\u0103rin\u021bilor de la v\u00e2rsta de 16 ani sau mai devreme, \u00een cazurile \u00een care p\u0103rin\u021bii nu pot fi contacta\u021bi \u0219i sunt necesare interven\u021bii de salvare a vie\u021bii.<\/li>\r\n \t<li><strong>Finan\u021barea sistemului de s\u0103n\u0103tate:<\/strong> Asigurarea de s\u0103n\u0103tate pentru copii, studen\u021bi \u0219i femei \u00eens\u0103rcinate este pl\u0103tit\u0103 de stat. Un pachet de baz\u0103 de servicii este disponibil gratuit pentru to\u021bi. Compania na\u021bional\u0103 de asigur\u0103ri \u00een medicin\u0103 finan\u021beaz\u0103 Centrele de S\u0103n\u0103tate Prietenoase Tinerilor (CSPT) din 2008, asigur\u00e2nd acces gratuit pentru to\u021bi tinerii cu v\u00e2rste cuprinse \u00eentre 10 \u0219i 24 de ani \u0219i pentru tinerii vulnerabili cu v\u00e2rsta de p\u00e2n\u0103 la 35 de ani.<\/li>\r\n \t<li><strong>Furnizarea serviciilor:<\/strong> Exist\u0103 o re\u021bea de 41 de Centre de S\u0103n\u0103tate Prietenoase Tinerilor \u2013 Youth Klinics (YK) \u00een institu\u021biile publice de asisten\u021b\u0103 medical\u0103 primar\u0103 din toate municipiile \u0219i raioanele Moldovei, pe baza standardelor de calitate ale serviciilor de s\u0103n\u0103tate prietenoase tinerilor, cu un accent special pe tinerii din mediul rural, adolescen\u021bii vulnerabili \u0219i \u00een situa\u021bii de risc. Acestea includ servicii medicale mobile (YK Mobile) \u0219i consiliere online (linia de asisten\u021b\u0103 YK).<\/li>\r\n \t<li><strong>For\u021ba de munc\u0103 din domeniul s\u0103n\u0103t\u0103\u021bii:<\/strong> Cursurile\/modulele de formare \u00een domeniul s\u0103n\u0103t\u0103\u021bii adolescen\u021bilor au fost \u00eencorporate \u00een programele de \u00eenv\u0103\u021b\u0103m\u00e2nt de la universitatea de medicin\u0103 \u0219i colegiile de asisten\u021b\u0103 medical\u0103 \u0219i moa\u0219e. Sesiunile de \u00eenv\u0103\u021bare prin colaborare sunt organizate \u00een mod regulat de toate CSPT\/YK.<\/li>\r\n \t<li><strong>Sisteme de informare \u00een domeniul s\u0103n\u0103t\u0103\u021bii:<\/strong> \u00cen plus, pe l\u00e2ng\u0103 sistemul na\u021bional de informare privind managementul s\u0103n\u0103t\u0103\u021bii, a fost dezvoltat\u0103 \u0219i implementat\u0103 o component\u0103 confiden\u021bial\u0103 separat\u0103 \u2013 Sistemul informa\u021bional pentru tineri \u2013 pentru CSPT\/YK pentru a monitoriza activit\u0103\u021bile acestora.<\/li>\r\n \t<li><strong>Produse medicale:<\/strong> Din 2017, produsele contraceptive \u0219i testele rapide pentru HIV, sifilis, hepatit\u0103 sunt procurate de stat \u0219i distribuite de medicii de familie, cabinetele de s\u0103n\u0103tate reproductiv\u0103, Centrele de S\u0103n\u0103tate Prietenoase Tinerilor. Achizi\u021bionarea de medicamente pentru avortul medicamentos a fost posibil\u0103 pentru adolescen\u021bi \u00eentr-un CSPT \u00eenc\u0103 din 2012, iar din anii 2022-2023 aceasta s-a l\u0103rgit \u00een toate Centrele de S\u0103n\u0103tate Prietenoase Tinerilor.<\/li>\r\n<\/ul>\r\n<strong>Rezultate<\/strong>\r\n\r\nMediul legislativ \u0219i politic puternic, \u00eempreun\u0103 cu consolidarea datelor \u0219i interven\u021biilor menite s\u0103 creasc\u0103 furnizarea de servicii au dus la \u00eembun\u0103t\u0103\u021birea rezultatelor privind s\u0103n\u0103tatea \u0219i drepturile sexuale \u0219i reproductive ale adolescen\u021bilor, \u00een ultimul deceniu. Accesul la CSPT pentru tinerii cu v\u00e2rste cuprinse \u00eentre 10 \u0219i 24 de ani a crescut de la 5% \u00een 2011 la 38% \u00een 2023. Eforturile de informare au dus la un num\u0103r mai mare de utilizatori vulnerabili ai serviciilor \u0219i de beneficiari de sex masculin care au accesat serviciile. Utilizarea pilulelor contraceptive \u00een r\u00e2ndul adolescen\u021bilor activi sexual a crescut de la 6% \u00een 2014 la 13% \u00een 2022, iar majoritatea adolescen\u021bilor activi sexual (74%) continu\u0103 s\u0103 foloseasc\u0103 prezervative. Rata fertilit\u0103\u021bii adolescentelor a sc\u0103zut de la 34,2 la 1000 de femei cu v\u00e2rste cuprinse \u00eentre 15 \u0219i 19 ani \u00een 2015 la 23 la 1000 de femei \u00een 2022. \u00cen acela\u0219i timp, rata de avort a sc\u0103zut de la 11,7 la 1000 de femei cu v\u00e2rste cuprinse \u00eentre 15 \u0219i 19 ani la 5,9\/1000. Inciden\u021ba HIV \u00een r\u00e2ndul persoanelor cu v\u00e2rsta cuprins\u0103 \u00eentre 15 \u0219i 24 de ani a r\u0103mas neschimbat\u0103 \u00een ultimul deceniu \u0219i este de peste dou\u0103 ori mai mic\u0103 dec\u00e2t inciden\u021ba HIV la popula\u021bia general\u0103.\r\n\r\n<strong>Provoc\u0103ri<\/strong>\r\n\r\nProvoc\u0103rile pentru Centrele de S\u0103n\u0103tate Prietenoase Tinerilor includ instabilitatea politic\u0103, social\u0103 \u0219i umanitar\u0103, subfinan\u021barea, accesul limitat pentru tinerii din zonele rurale \u0219i mecanismele insuficiente pentru a motiva personalul medical din spitale \u0219i medicii de familie s\u0103 aplice abordarea prietenoas\u0103 adolescen\u021bilor care caut\u0103 \u00eengrijiri medicale. Mai mult, subdezvoltarea serviciilor de s\u0103n\u0103tate \u0219colar\u0103 \u0219i lipsa implement\u0103rii unui program complex de educa\u021bie pentru s\u0103n\u0103tate prezint\u0103 obstacole \u00een asigurarea accesului universal la servicii de s\u0103n\u0103tate prietenoase pentru adolescen\u021bii din Republica Moldova.\r\n\r\n<strong>Lec\u0163ii<\/strong>\r\n<ul>\r\n \t<li>Reformele legislative \u0219i politice au extins gama de servicii pe care adolescen\u021bii \u0219i tinerii le puteau accesa, elimin\u00e2nd barierele structurale critice \u0219i extinz\u00e2nd acoperirea geografic\u0103.<\/li>\r\n \t<li>Reducerea taxelor de servicii pentru adolescen\u021bi, \u00een special \u00een cadrul pachetelor de finan\u021bare \u0219i beneficii, \u0219i accesul la servicii f\u0103r\u0103 consim\u021b\u0103m\u00e2ntul p\u0103rin\u021bilor, ajut\u0103 la eliminarea barierelor privind s\u0103n\u0103tatea \u0219i drepturile sexuale \u0219i reproductive ale adolescen\u021bilor.<\/li>\r\n \t<li>Integrarea acestor servicii \u00een institu\u021biile de formare medical\u0103 existente ajut\u0103 la construirea unui cadru mare de furnizori care ar putea oferi adolescen\u021bilor \u0219i tinerilor servicii f\u0103r\u0103 a fi nevoie de formare specializat\u0103.<\/li>\r\n \t<li>Integrarea s\u0103n\u0103t\u0103\u021bii \u0219i drepturilor sexuale \u0219i reproductive ale adolescen\u021bilor \u00een pachetele esen\u021biale ale CSPT a creat un \"ghi\u0219eu unic\" pentru serviciile cheie de s\u0103n\u0103tate sexual\u0103 \u0219i mintal\u0103 pentru adolescen\u021bi.
